Please fill out all fields below to register for the Maplestix Player's Clinic, taking place Saturday December 11th, 2021 at the Pickering Soccer Club at 1975 Clements Road, in Pickering, Ontario. Once all fields are completed you are asked to e-transfer your payment ($141.25)
Cost of the Maplestix Player's Clinic is $ 125.00+tax = $141.25.
Payment must be received upon registration through E-Transfer payment.
Send your payment via E-Transfer for $141.25 ($125+Tax) and set-up a simple password if needed for MapleStix to use to james.calder2@sympatico.ca.

The Player's Clinic is limited to 60 participants and includes only 6 Goalie spots - so sign-up today to ensure your registration.
You are not officially registered until your E-Transfer is processed. Once your registration through E-Transfer is paid you will receive confirmation and instructions via email.
Public Health Restrictions limit numbers that can attend. Only Players, Coaches and MapleStix Employees will be allowed entrance into the Dome.
• Refunds available only if Covid Related Issues Cause Cancellation of Event. Each player will receive the
majority of the registration fee minus an administrative portion.
